Why these four, and why they still lead
The Fatal Four earn the name from the data. Across OSHA and BLS reporting, these four causes consistently account for well over half of all construction worker deaths in a given year. Falls alone are the single largest category — the leading cause of construction fatalities and OSHA’s most-cited standard, year after year. The other three trade places behind it, but the group as a whole is remarkably stable, which tells you something important: these are not freak accidents. They are the predictable result of the same handful of controls being skipped.

1. Falls — the leading killer, and the top citation
Falls from height are the defining hazard of construction. On a real site they look like a worker on a leading edge with no guardrail, someone on a roof without a personal fall arrest system tied off to a real anchor, a floor or roof hole without a secured cover, an unguarded scaffold platform, or a ladder set wrong and used wrong. The fall protection trigger in construction is generally 6 feet above a lower level — above that, you need guardrails, a personal fall arrest system, or a safety net, selected for the work.
Falls stay on top for a reason that has nothing to do with difficulty: the controls exist and are well understood, but they get skipped for speed on a “quick” task. The two-minute roof trip with no tie-off is the one that kills. Because falls are the leading fatality cause and the most-cited standard, they are also where OSHA’s targeted fall-hazard enforcement is most active — and where documented daily attention pays off most.
2. Struck-by — the hazard hiding in normal work
Struck-by injuries come from objects and equipment in motion: a load swinging off a crane or forklift, materials falling from above, a nail or fragment from a tool, and — one of the deadliest — vehicles and mobile equipment striking a worker on foot. On the ground, struck-by looks like a crew working under a suspended load, a spotter who isn’t there when a truck backs up, unsecured material stacked near an edge, or a worker in the swing radius of an excavator.

3. Caught-in/between — trenches, machinery, and pinch points
Caught-in/between kills when a worker is trapped, crushed, or squeezed — between equipment and a fixed object, in unguarded machinery, or, most notoriously, in a collapsing trench. A cubic yard of soil weighs on the order of a small car; a trench wall that fails buries a worker faster than anyone can pull them out, which is why trenching sits at the center of this category and at the center of current enforcement.
On site, caught-in/between looks like an unprotected excavation five feet or deeper with no sloping, shoring, or shield; a worker between a swinging counterweight and a wall; or an unguarded rotating shaft or pinch point on machinery. Trenching in particular has drawn recent fatal-incident citations and remains a targeted enforcement priority — a competent person, a protective system, and a safe way in and out are non-negotiable.
4. Electrocution — overhead lines, contact, and bad temporary power
Electrocution deaths come from contact with energized sources: overhead power lines struck by cranes, ladders, or scaffolds; damaged cords and tools; missing GFCIs on temporary power; and work on circuits that should have been de-energized and locked out. Overhead line contact is a leading killer in this category because the clearance gets misjudged during a lift or a move.

Here is the encouraging part: because the Fatal Four are so consistent, the prevention is consistent too. You do not need a new program for each hazard every morning. You need a short, honest daily conversation that names the day’s exposures and the controls, and a record that it happened.
- Name the day’s Fatal Four exposures out loud at the morning briefing — who’s working at height, near equipment, in an excavation, or around power today.
- Confirm the controls are in place before work starts, not after — guardrails and tie-offs, spotters and exclusion zones, protective systems and safe egress, GFCIs and clearances.
- Assign the competent person for the tasks that require one — fall protection, excavation, scaffolding — and make sure the crew knows who it is.
