Inspection: before each use, after every incident
The foundation of 1926.1053 is that a ladder in poor condition never goes into service. The standard requires that ladders be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use — a tip-over, a dropped load, contact with a vehicle, anything that could have damaged it. In practice, that means the smart, expected habit is a quick look before each use, backed by the competent-person inspections the rule requires.
A ladder that has been knocked over or otherwise involved in an incident must be pulled and inspected before it is used again — you do not just stand it back up and keep climbing. What the inspection looks for is straightforward:
- →Rails and rungs — cracks, bends, splits, dents, or corrosion on side rails; loose, missing, or bent rungs and steps.
- →Feet and safety shoes — worn, missing, or damaged non-slip feet that let the base kick out.
- →Hardware — loose or missing bolts, rivets, and spreaders; on extension ladders, worn rope, damaged pulleys, and rung locks that don’t seat.
- →Contamination — grease, mud, wet paint, or ice on rungs that destroy footing.
- →Structural integrity of the whole ladder — anything that makes you hesitate is a reason to pull it.
Setup: the 4:1 angle and the 3-foot extension
Two setup numbers cause more ladder citations than anything else, and both are easy to hit once you know them.
The 4-to-1 rule
A portable, non-self-supporting ladder (an extension or straight ladder that leans against a surface) must be set so that the horizontal distance from the base to the wall is about one-quarter of the working length of the ladder to that support point. Put simply: for every four feet up, move the base one foot out. Too steep and the ladder tips backward when you climb; too shallow and the base kicks out from under you. The quick field check — stand with your toes at the feet, arms straight out, and your palms should just reach the rungs.
The 3-foot extension above the landing
When a ladder is used to access an upper landing surface, the side rails must extend at least 3 feet above that landing. If they can’t, the ladder has to be secured at the top and a grasping device (like a grab rail) provided so a worker can get on and off without lunging. The 3-foot extension is what gives a person something to hold as they transition at the top — the moment ladder falls most often happen.
- →Set the ladder on firm, level footing — never on boxes, blocks, or unstable bases to gain height.
- →Secure the top, and the bottom where it could shift; on a slick surface, tie it off or have someone hold it.
- →Keep the area around the top and base clear, and never set a ladder in front of a door that opens toward it unless the door is blocked or guarded.
- →Use the right ladder for the environment — non-conductive (fiberglass) ladders near electrical work; metal ladders never around energized lines.
Climbing: 3-point contact and facing the ladder
The rule for climbing is simple and non-negotiable: maintain three points of contact — two hands and a foot, or two feet and a hand — and face the ladder the entire time. That is impossible if a worker is carrying material up by hand, which is why loads that could cause a loss of balance are not allowed on the climb. Hoist tools and materials separately, or use a tool belt, so both hands stay free to grip.
- →Face the ladder going up and down — never climb or descend facing outward.
- →Keep your body centered between the rails; don’t overreach. Belt buckle stays between the side rails. If you have to lean out, get down and move the ladder.
- →Never stand on the top step or top cap of a stepladder, and don’t use the top few rungs of an extension ladder as a standing surface.
- →One person per ladder unless it is specifically designed for more.
Defective ladders: tag it, pull it, don’t “make it work”
When a ladder is found defective, 1926.1053 is explicit: it must be immediately marked or tagged “Do Not Use” (or otherwise clearly identified) and withdrawn from service until repaired or destroyed. A damaged ladder cannot stay in the rotation because it’s the only one on the truck. The most common version of this violation is a ladder with a cracked rail or a missing foot still leaning against the building because nobody pulled it.
Repairs, when made, have to restore the ladder to a condition that meets its original design — you don’t field-splice a cracked rail with a scrap of two-by-four and call it good. If it can’t be properly repaired, destroy it so it can’t find its way back into use. The tag-and-remove step is cheap; the fall it prevents is not.

Every one of these is preventable in the sixty seconds before the climb. That is the real message of 1926.1053: ladder safety is not a specialized skill, it is a habit of looking — at the ladder, at the angle, at the landing — every single time. Because a ladder fall is one of the ways falls stay at the top of the fatality list, and falls are precisely where OSHA’s targeted enforcement continues to focus in 2026.
